Patient Information

PHONES:

  • Phones are answered from:
    • 7:30AM to12:30PM and 1:00PM to 4:30 PM        Monday – Friday 
    • 9:00AM-Noon on Saturday

 Our phones are centralized and based at our main location (Deer Lakes Office) the phone # is 412-767-5387

 

  • We do not have direct calling lines to our other office location.  The operator is able to transfer your calls to speak to our receptionist or nurse at those locations.
  • Nurses are available to take return phone calls after 9:00AM.

APPOINTMENTS:

 

  • When calling for an appointment please specify what location you go to:
    • Cheswick – located on Russellton Dorseyville Road
    • Fox Chapel – located on Old Freeport Road
    • Oakmont – located on Allegheny River Blvd.

 

  • Blood work is scheduled 8:30-9:00AM, Monday – Friday at all locations.

 

  • Please help us provide better service by keeping your scheduled appointment.  We may call 24-48 hours in advance to remind you of your scheduled office appointment. 

 

  • Please notify us at least 24 hours in advance if you are unable to keep your scheduled appointment.  We may charge a $25.00 fee for missed, cancelled, or rescheduled appointments without a 24-hour notice.  Repeated failure to keep appointments may jeopardize your next visit to our office.

 

COMPLETE PHYSICAL EXAMS:

 

  • Complete Physical Exam is schedule every day at 9:00AM and 11:00 AM.  The physical consists of a physical exam by the physician, EKG, lab work and a urine test.  You should call your insurance co. before arriving at the office to make sure your policy covers Preventive Health Care Exams, EKG and Lab work.  You will be asked to sign a waiver of responsibility that any procedures that are not covered by you insurance you will be responsible for.

 

Complete Physical Exams must be cancelled with in 24 hours or you will be charged $25.00 for a missed or cancelled appointment.

 

MINOR PATIENT (UNDER 18 YEARS OF AGE):

 

It is a Penna. State Law that a legal guardian must accompany children under the age of 18 at the time of visit.  The legal guardian may write a note and send it with the child to be seen along with a responsible adult.  No injections can be given to a child under the age of 18 unless the legal guardian is with the child to sign for the injection.

 

PRESCRIPTIONS:

 

  • When you call for a refill on a prescription please make sure you have your bottle in front of you. We will need:
  • Pharmacy name and phone number.
  • The name of the medication
  • The strength (mg)
  • The dose – how you take the medication (1 every day, 2 every day etc.)
  • The quantity (do you need a 90 day supply – do you use mail orders)
  • Please give us a minimum of 24 hour to refill prescription.
  • Authorization for refills and number of refills is at your physician’s discretion and should be discussed with him/her.

 INSURANCE:

 

  • Your insurance policy is a contract between you and your insurance company.  We will submit medical services to your insurance company if we are a participating provider, and if you have given us all of the required information.  We must have the most current copy of your insurance card.  You must notify us immediately of any change in your insurance coverage.  Please be aware that some, and perhaps all of the services provided, may be considered “non-covered services” according to your policy.  You will be responsible for payment of these services.  If we are non-participating providers with your insurance company, you will be responsible for charges at the time of service.  You can submit your paid receipt to the insurance company for reimbursement.

 

  • We are participating with most major insurance companies.  Please check with our office staff for the current list of these companies.  At the time of service, we will collect the co-payment indicated on your insurance card.  You will be responsible for payment of any deductibles, co-insurance or non-covered services.

 

BILLING:

 

If you have a financial problem, please ask to discuss a payment plan with our Billing Office.  A billing statement of medical services received will be mailed to you on a monthly basis.  If an extended payment plan has been offered to you, we require regular monthly payments or the plan is void.  After that, the account may be turned over to our collection agency and possible termination of services.  We accept cash, checks, Visa/MasterCard for payment.  In the event a personal check is returned, unpaid from the bank for any reason, your account will be charged with a $20.00 return check fee.

 

Physician Assistant:

 

Q. What is a Physician Assistant(PA)? 

Physician assistants are health care professionals licensed to practice medicine with physician’s supervision.  As part of their comprehensive responsibilities, PAs conduct physical exams, diagnose and treat illnesses, order and interpret tests, counsel on preventive health care, assist in surgery, and can write prescriptions.  Within the physician-PA relationship, physician assistants exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services.

 

PAs are trained in intensive education programs accredited by the Accreditation Review Commission on Education for the Physician Assistant.

 

Because of the close working relationship the PAs have with physicians, PAs are educated in the medical model designed to complement physician training.  Upon graduation, physician assistants take a national certification examination developed by the National Commission on Certification of PAs in conjunction with the National Boarded of Medical Examiners.  To maintain certification, PAs must log 100 hours of continuing medical education every two years and sit for a recertification every six years.  Graduation from an accredited physician assistant program and passage of the national certifying exam are required for state licensure.

  

Patients Responsibilities

 

  • Must bring Insurance Card to every visit.
  • Must pay co-pay at every visit – we also accept Visa and Mastercard.
  • Please bring a list of all medications you are currently taking to your appointment.
  • Patient must call their Insurance Co and verify coverage for Complete Phys. Exams.
  • Patient must stop at the front desk when leaving the office to:       
    • Make a return appointment
    • If referral is needed to see a specialist – this can be done at the front desk when leaving.
    • Make sure you have all new written prescriptions before leaving the office.
